I also think that Chat Moderators do this pretty well. Most moderators will just work in servers like normal players would, and share their experiences. The trouble is that there's approximately 35 moderators on the servers and several thousand players at a time. It is just physically impossible for Chat Moderators to work in every server, or even in most servers. So, while we agree that in an ideal world we'd have a moderator work in every server it's just not feasible to do so.

 That is why we have the Chat Violations section. Players can alert Chat Moderators to trouble that we might not see, and allows them to react to things.

 

As @DonCarnage said, there is no enough moderator to keep the chat clean. 

My suggestion is a /vote like system, in connection with chat. 

The players can use a chat command / click a button to alert active moderators. If (for example) 5 or more player alert the moderators in one server in a minute, the moderator would check out the server's chat.

Please note:

-Only works on the public chats (it means there is about 30-35 chat where the alerts come from)

-The minimum number of players (5) is just an example, depends on the developers opinion.

-A player can not alert two times in (example!) 5 minutes. So, if you used the command/button you won't be able to flood the moderators with your alerts for 5 minutes.

Chat command: /alert

Button at the bottom of the chat window: [Alert moderators]

 

Why do I suggest this: It would help the moderators to avoid scams/serious violations in real time.

There is generally a moderator in all servers sneaking to find a violator. Although if the moderator missed it, the player can easily take a screenshot and post it in the chat violations section. In addition to that players who don't know how to post such commands with the "/" will flood chats with why isn't it happening.
