Need some help with nickname change

I opened the nickname change form, and at the end it said ''Please attach screenshots''. I am not sure what screenshots I have to give, do i have to give a screenshot of the homepage of my account? 

 

And do I need to prove that this is my account?

 

Thanks, pls answer my question as soon as possible.

It is not required to attach screenshots in the application. As long as you answer those required questions correctly, you'll be fine.

To be clear, questions with the symbol "V1KbDr0.png" means it is required to be answered correctly to prove you are the owner of the account. Failing to answer those will end up having your application rejected.

So what do i write where it asks about my internet providers and where i play the game?

Simply state where (meaning places) you've played the game at. Also, state all internet providers that you've accessed throughout your whole gaming experience here. To be specific, internet providers are organizations that provides services accessing and using the Internet, meaning the company/organization that provides your internet connection.

You write all the places (cities, countries) where you accessed the game. For internet providers you write the name of the company (or companies) that you buy internet from.

Yes, it is. The more information you provide, the more chances that your application will be successful. If you don't provide enough info there's a risk that tech support (the guys who change your name) will decide that you didn't provide enough evidence to prove that you are the owner of the account for which you want to change name.

By attachments/screenshots, I think they mean this:

About the other stuff, the more information you give, the faster your application is processed. All of the questions are mandatory, anyway.

Yeah that's where you need to attach any screenshots required for previous questions.
