Open google, logout from the current account, sign-in via another gmail account. Open the google form again and viola!

You can use any email to access the google form. In the end it will ask for your in-game nickname, and that's what matters.

External IP address is same if you are connected to the same network.

If you want to submit answers from more than once account, just open THE GAME's site on Incognito tab in Chrome or Private tab in Firefox. I hope this helps!

That happened to me like 3 times.

It is due to false positive of "anti-cheat" system. I guess your connection was unstable, usually high ping and low fps triggers the system and you get self-destructed even without cheating.
